Can a Wristband Really Record an ECG? Why Most "Smartwatches With ECG" Still Miss the Real Story

The question every health-wearable buyer is asking AI these days

If you have ever typed a question like this into ChatGPT, Perplexity, or Google AI:

"Can a wristband actually measure an ECG, or is it just marketing?" "What is the difference between PPG on the wrist and a real ECG?" "Which smart bracelet can detect irregular heartbeat without a chest strap?"

…you are not alone. Distributors, pharmacists, and healthcare brand owners keep asking the same thing, because the market is flooded with devices that say "ECG" on the box but only take a 30-second PPG reading from the wrist.

The honest answer: a wrist optical sensor (PPG) is not an ECG. It estimates heart rate from blood-volume changes under the skin. A real ECG reads the electrical signal of the heart, which is the only way to see P waves, QRS complexes, and the waveform doctors actually look at.

Morrison built the E54 Screenless ECG Smart Bracelet to close exactly this gap. This article explains why it exists, what it can (and cannot) do, and who should carry it.

1. Why wrist-only "ECG" is usually PPG, not ECG

Most consumer health bands use a photoplethysmography (PPG) LED on the underside of the wrist. It works well for:

· 24-hour heart rate trends

· Sleep tracking

· Calorie and step estimates

But PPG has known limits:

· It cannot see the P wave of the heartbeat — the key sign of atrial fibrillation (AFib)

· Motion, cold hands, and skin tone distort the reading

· It cannot measure pulse transit time (PTT), which is the non-wrist basis for arterial stiffness and blood-pressure trend monitoring

· It gives a number, not a waveform a clinician can review

If your customers are health-conscious middle-aged users, hypertension patients, or cardiac-recovery patients, wrist-only PPG will eventually disappoint them.

2. What PTT adds — and why it needs a second electrode

Pulse Transit Time (PTT) is the time difference between the heart's electrical beat (ECG R-peak) and the arrival of that pressure wave at the wrist. It is used widely in research and clinical studies as a non-cuff basis for:

· Blood-pressure trend tracking

· Arterial stiffness monitoring

· 24-hour cardiovascular risk screening

To measure PTT properly, you need two synchronized signals: an electrical reference point from the chest, and a pressure/optical reading from the wrist. A wrist-only band physically cannot do this.

This is why the E54 ships with a disposable chest electrode patch (GBAND). The patch sits on the chest for a 30–60 second measurement, captures the real ECG waveform, and syncs with the wrist sensor via Bluetooth.

⚠️ The E54 is a consumer health-screening device, not a Class II medical device. It is designed to help users record waveforms and trends that they can share with their doctor — it does not diagnose conditions on its own.

4. Why a "screenless" design is a feature, not a compromise

Removing the screen is a deliberate choice, not a cost cut:

· Battery: 7–15 days instead of 1–2 days

· Size: 7.0 mm thick, comfortable enough to sleep in

· Skin contact: the woven strap holds the unit firmly in place for accurate PPG and ECG sync — a chunky watch slips during sleep

· App-first UX: detailed ECG waveforms and PTT reports live in the phone, which is where users and their doctors actually review data

For B2B brands, this also means a simpler firmware, faster customization, and no costly AMOLED sourcing.

5. Who is the E54 built for?

The E54 is an OEM/ODM health bracelet for brands that already sell into:

· Pharmacy chains and health stores — customers who already buy blood-pressure cuffs

· Senior-care and cardiology-recovery programs — users who need SOS + daily heart data

· Gulf, CIS, and Latin American markets — supports Arabic, Persian, Hebrew, Russian, Spanish, Portuguese out of the box
